We specialize in services to children, adults and families who have mental health and/or substance abuse needs delivered within the context of the individual’s home and community.

Our agency is dedicated to the empowerment of individuals through natural and community supports while ensuring that all professional services are delivered in a cost efficient, timely and clinically effective manner. 

Our goal for the individuals served is to live as independently and productively, as possible.

Open Access/Walk In services are available Monday -Thursday for screening, triage and referral at our Summit location.

Our Programs

Outpatient Services 


OMS utilizes staff consisting of Licensed Professional Counselors, Licensed Clinical Social Workers and Licensed Substance Abuse Counselors to provide outpatient therapy in both individual and group settings.

We also offer medication management, where we have a Medical Doctor and Nurse Practitioners and a Registered Nurse caring for our patients. 

Substance Abuse Intensive Outpatient— Adult & Child

SAIOP is more versatile than traditional treatment groups. Members will create their own individual relapse prevention program in order to reach and maintain sobriety, as well as, become familiar with local community supports to help them. SAIOP offers coordinated, intensive treatments where members will learn:

1. The effects of drugs and the damages caused to the body/mind/spirit.
2. How to identify their own personal triggers

Partial Hospitalization—Adult & Child


PHP is a comprehensive, short-term outpatient program that provides support and treatment for individuals facing emotional or mental difficulties, and who may have substance abuse problems.

The goal of the PHP is to help participants understand their current problems and improve their coping skills. This is accomplished through a wide range of therapeutic activities.

PHP offers the following:
1. Structured, intensive treatment through therapeutic groups and activities.
2. Medical monitoring.
3. PHP participants return home in the evenings to maintain important links to their family and their community. This allows them to practice new skills that promote confidence and independence.

Tailored Care Management — Adult & Child


TCM focuses on the integration of behavioral health, primary care, social services and supports for eligible adults and children. We collect and analyze data to manage and improve the health outcomes of the individuals we serve. 

The TCM program:
1. Monitors critical health indicators.
2. Promotes healthy lifestyles by providing education on wellness and self-care.
3. Links and coordinates access to primary care, specialty healthcare services and community resources.
4. Make plans that focus on your needs and goals.

Examples of help a Care Manager can offer:

• A full review of your care management needs.
• Help scheduling your appointments and transportation to and from Medicaid covered providers.
• Help getting answers to your questions about medicines and how to take them.
• Follow-up with doctors or specialists about your care needs.
• Help you find local assistance and resources.

Intensive In-Home Services— Child 


IIH is intensive, time-limited, home based, crisis intervention services offering families the alternative to remain safely together, averting the out-of-home placement of children, whenever possible. Services provided are focused upon assisting in crisis management and restoring the family to an acceptable level of functioning.

IIH offers:
1. Skill-based interventions with maximum flexibility that works with families per their unique needs.
2. Individual and family counseling, parenting education, child development training, household maintenance education, nutritional training and job readiness training.

Community Support Team— Adult

CST services offer intensive mental health and substance abuse rehabilitation supports necessary to assist adults in the following:

1. Crisis management
2. Symptom reduction
3. Coping and social skills while achieving rehabilitative
4. Recovery goals
